Hatfield Primary Care Network has been presented with the Community Champion Award by High Sheriff of Hertfordshire Annie Brewster.
The team were given the award for outstanding service at their education afternoon on Tuesday last week.
Annie Brewster has adopted Sport for Good as the theme for her year in office, and Hatfield PCN has introduced a number of initiatives in the last 18 months for patients – working in partnership with local charities, Welwyn Hatfield Borough Council and the Herts Sports & Physical Activity Partnership.
These initiatives include running a healthy eating hub, setting up exercise and wellbeing programmes, delivering health walks with patients and working with Hatfield patient groups to organise clinical education evenings.
Sarah Ellingworth, Hatfield PCN manager, said: "This award recognises the hard work and commitment of our dedicated staff who work tirelessly to improve the health and wellbeing of our patients. They are such a brilliant team."
Hatfield PCN has also focused on improving the health and wellbeing of their own staff and initiatives have included a 5k walk, chair yoga and most recently a Big Steps Challenge – where practice teams competed to walk as many steps as possible in September.
